You may preregister with our office by filling out our Adult Patient Medical History or our Child Medical History Form.
After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information.
On your first visit to our office, we will have your completed form available for your signature.
You may also Request an Appointment. All you need to do is give us the time and date most suited to you.
We will find an appointment time and date that is as close as possible to your request and then be in touch with you on the contact information you give us to confirm your appointment.
After your visit to our clinic, please take the time to fill out the Patient Satisfaction Survey.
This will help us to continually improve our services to you.
